The field of neuroaesthetics attempts to identify the brain processes underlying aesthetic experience, including but not limited to beauty. Previous neuroaesthetic studies have focussed largely on paintings and music, while performing arts such as dance have been less studied. Nevertheless, increasing knowledge of the neural mechanisms that represent the bodies and actions of others, and which contribute to empathy, make a neuroaesthetics of dance timely. Here, we present the first neuroscientific study of aesthetic perception in the context of the performing (...) arts. We investigated brain areas whose activity during passive viewing of dance stimuli was related to later, independent aesthetic evaluation of the same stimuli. Brain activity of six naïve male subjects was measured using fMRI, while they watched 24 dance movements, and performed an irrelevant task. In a later session, participants rated each movement along a set of established aesthetic dimensions. The ratings were used to identify brain regions that were more active when viewing moves that received high average ratings than moves that received low average ratings. This contrast revealed bilateral activity in the occipital cortices and in right premotor cortex. Our results suggest a possible role of visual and sensorimotor brain areas in an automatic aesthetic response to dance. This sensorimotor response may explain why dance is widely appreciated in so many human cultures. (shrink)

Background: Sharing information with relatives of elderly patients in primary care and in hospital has to fit into the complex set of obligations, justifications and pressures concerning the provision of information, and the results of some studies point to the need for further empirical studies exploring issues of patient autonomy, privacy and informed consent in the day-to-day care of older people.Objectives: To know the frequency with which “capable” patients over 65 years of age receive information when admitted to hospital, the (...) information offered to the families concerned, the person who gives consent for medical intervention, and the degree of satisfaction with the information received and the healthcare provided.Method: A descriptive questionnaire given to 200 patients and 200 relatives during the patients’ stay in hospital.Results: Only 5% of patients confirmed that they had been asked whether information could be given to their relatives. A significantly higher proportion of relatives received information on the successive stages of the care offered than did patients themselves. As the age of the patients increased, so the number who were given information, understood the information and were asked for their consent for complementary tests decreased. The degree of satisfaction with the information offered was high for both patients and relatives , despite the irregularities observed.Conclusions: The capacity of elderly patients to participate in the decision-making process is frequently doubted simply because they have reached a certain age and it is thought that relatives should act as their representatives. In Spain, the opinion of the family and doctors appears to play a larger role in making decisions than does the concept of patient autonomy. (shrink)

In Spain, any person under the age of 18 is a minor. Generally, minors lack the legal capacity to take legally binding actions because they are deemed incapable of legally binding consent. Spanish civil law recognises, however, that the child, in accordance with the law and being sufficiently mature, may act for himself. It stands, then, that consent, as expressed by the “sufficiently mature” minor, should be legally valid.

Sacrificial dilemmas such as the trolley problem play an important role in experimental philosophy (x-phi). But it is increasingly argued that, since we are not likely to encounter runaway trolleys in our daily life, the usefulness of such thought experiments for understanding moral judgments in more ecologically valid contexts may be limited. However, similar sacrificial dilemmas are experienced in real life by animal research decision makers. As part of their job, they must make decisions about the suffering, and often the (...) death, of many non-human animals. For this reason, a context-specific investigation of so-called “3R dilemmas” (i.e., dilemmas where there is a conflict between the principles of replacement, reduction, and refinement of the use of animals in research) is essential to improve the situation of both non-human animals and human stakeholders. An approach well suited for such investigation is experimental philosophical bioethics (“bioxphi”), which draws on methods similar to x-phi to probe more realistic, practical scenarios with an eye to informing normative debates and ethical policy. In this article, we argue for a need to investigate 3R dilemmas among professional decision-makers using the tools of bioxphi. In a first step, we define 3R dilemmas and discuss previous investigations of professionals’ attitudes in such cases. In a second step, we show how bioxphi is a promising method to investigate the whys and hows of professional decision-making in 3R dilemmas. In a last step, we provide a bioxphi template for 3R dilemmas, give recommendations on its use, explore the normative relevance of data collected by such means, and discuss important limitations. Background: Medical confidentiality underpins the doctor–patient relationship and ensures privacy so that intimate information can be exchanged to improve, preserve, and protect the health of the patient. The right to information applies to the patient alone, and, only if expressly desired, can it be extended to family members. However, it must be remembered that one of the primary tenets of family medicine is precisely that patient care occurs ideally within the context of the family. There may be, then, certain occasions (...) when difficulties will arise as to the extent of the information provided to family members.Objectives: This study aimed to describe family doctors’ attitudes to confidentiality and providing patient information to relatives as well as their justifications for sharing information.Method: A descriptive postal questionnaire was self-administered by family doctors.Results: Of 227 doctors, 95.1% provided information to a patient’s family and over a third disclosed information to others without prior patient consent.Conclusions: The findings reveal that family doctors should pay more attention to their patients’ rights to information, privacy, and confidentiality, and reflect very carefully on the fine balance between this and the occasional need for the support and collaboration of family members in delivery of care. Emphasis should be placed on ethics and legal problems during undergraduate education and in-service training of doctors. (shrink)

A large part of technological advances, especially in the field of industry, have been focused on the optimization of productive processes. However, the detection of anomalies has turned out to be a great challenge in fields like industry, medicine or stock markets. The present work addresses anomaly detection on a control level plant. We propose the application of different intelligent techniques, which allow to obtain one-class classifiers using real data taken from the correct plant operation. Nowadays, batteries play an important role in a lot of different applications like energy storage, electro-mobility, consumer electronic and so on. All the battery types have a common factor that is their complexity, independently of its nature. Usually, the batteries have an electrochemical nature. Several different test are accomplished to check the batteries performance, and commonly, it is predictable how they work depending of their technology. The present research describes the hybrid intelligent system created to accomplish fault detection over a (...) Lithium Iron Phosphate—LiFePO4 power cell type, commonly used in electro-mobility applications. The approach is based on the cell temperatures behaviour for voltage and current specific values. Taken into account the operating range of a real system based on a LiFePO4 cell, a large set of points of operation have been used to achieve the dataset. The different behaviour zones have been obtained by clustering as a first step. Then, different regression techniques have been used over each cluster. Polynomial regression, artificial neural networks and support vector regression were the combined techniques to develop the hybrid intelligent model proposed. The intelligent system gives very good results over the operating range, detecting all the faults tested during the validation. (shrink)

Automatic control of physiological variables is one of the most active areas in biomedical engineering. This paper is centered in the prediction of the analgesic variables evolution in patients undergoing surgery. The proposal is based on the use of hybrid intelligent modelling methods. The study considers the Analgesia Nociception Index to assess the pain in the patient and remifentanil as intravenous analgesic. The model proposed is able to make a one-step-ahead prediction of the remifentanil dose corresponding to the current state (...) of the patient. The input information is the previous remifentanil dose, the ANI variable and the electromyogram signal. Modelling techniques used are Artificial Neural Networks and Support Vector machines for Regression combined with clustering methods. Both training and validation were done with a real dataset from different patients. Results obtained show the potential of this methodology to calculate the drug dose corresponding to a given analgesic state of the patient. (shrink)

One of the main challenges in medicine is to guarantee an appropriate drug supply according to the real needs of patients. Closed-loop strategies have been widely used to develop automatic solutions based on feedback variables. However, when the variable of interest cannot be directly measured or there is a lack of knowledge behind the process, it turns into a difficult issue to solve. In this research, a novel algorithm to approach this problem is presented. The main objective of this study (...) is to provide a new general algorithm capable of determining the influence of a certain clinical variable in the decision making process for drug supply and then defining an automatic system able to guide the process considering this information. Thus, this new technique will provide a way to validate a given physiological signal as a feedback variable for drug titration. In addition, the result of the algorithm in terms of fuzzy rules and membership functions will define a fuzzy-based decision system for the drug delivery process. The method proposed is based on a Fuzzy Inference System whose structure is obtained through a decision tree algorithm. A four-step methodology is then developed: data collection, preprocessing, Fuzzy Inference System generation, and the validation of results. To test this methodology, the analgesia control scenario was analysed. Specifically, the viability of the Analgesia Nociception Index as a guiding variable for the analgesic process during surgical interventions was studied. Background: By providing healthcare to adolescents, a major opportunity is created to help them cope with the challenges in their lives, develop healthy behaviour and become responsible healthcare consumers. Confidentiality is a major issue in adolescent healthcare, and its perceived absence may be the main barrier to an adolescent seeking medical care. Little is known, however, about confidentiality for adolescents in primary care practices in Spain.Objective: To ascertain the attitudes of Spanish family doctors towards the right of adolescents to confidentiality (...) in different healthcare situations and in the prescription of treatment.Method: A descriptive postal questionnaire was self-administered by family doctors.Results: Parents of patients under 18 years are always informed by 18.5% of family doctors, whereas parents of those under 16 years are informed by 38.8% of doctors. The patients are warned of this likelihood by 79.3% of doctors. The proportion of doctors supporting confidentiality for adolescents increases with the age and maturity of the patients, whereas workload and previous training has a negative effect.Conclusions: Spanish laws on adolescent healthcare are not reflected by the paternalistic attitude that Spanish primary care doctors have towards their adolescent patients. Doctors need to be provided with up-to-date and clinically relevant explanations on contemporary legal positions. In primary care, more attention should be paid to adolescents’ rights to information, privacy and confidentiality. Doctors should be more aware of the need to encourage communication between teenagers and their parents, while also safeguarding their patients’ rights to confidential care. (shrink)

In this study, a hybrid model based on intelligent techniques is developed to predict the active power generated in a bioclimatic house by a low power wind turbine. Contrary to other researches that predict the generated power taking into account the speed and the direction of the wind, the model developed in this paper only uses the speed of the wind, measured mainly in a weather station from the government meteorological agency (MeteoGalicia). The wind speed is measured at different heights, (...) against the usual measurements in others researches, which uses the wind speed and the direction measured in a weather station on the wind turbine nacelle. The prediction is performed 30 minutes ahead, what ensures that the Building Management System knows the energy generated by the low power wind turbine 30 minutes before, and it can adapt the consumption of different equipment in the house to optimize the power use. The main objective is to allow the Building Management System to optimize the uses of energy, taking into account the predicted amount of energy that will be produced and the energy consumed in the house. The developed model uses a hybrid topology with four clusters to improve the prediction, achieving an error lower than 6.5% for Mean Absolute Error measured in a final test. To perform this test, part of the original dataset was isolated from the beginning of the training process to check the model with a dataset that is not used before, simulating the model as it is receiving new data. Closed-loop administration of propofol for the control of hypnosis in anesthesia has evidenced an outperformance when comparing it with manual administration in terms of drug consumption and post-operative recovery of patients. Unlike other systems, the success of this strategy lies on the availability of a feedback variable capable of quantifying the current hypnotic state of the patient. However, the appearance of anomalies during the anesthetic process may result in inaccurate actions of the automatic controller. These anomalies may come from the (...) monitors, the syringe pumps, the actions of the surgeon or even from alterations in patients. This could produce adverse side effects that can affect the patient postoperative and reduce the safety of the patient in the operating room. Then, the use of anomaly detection techniques plays a significant role to avoid this undesirable situations. This work assesses different one-class intelligent techniques to detect anomalies in patients undergoing general anesthesia. Due to the difficulty of obtaining real data from anomaly situations, artificial outliers are generated to check the performance of each classifier. The final model presents successful performance. The present study aims to explore whether the symptoms associated with fibromyalgia are contextually influenced by the area of residence. Furthermore, it is analyzed whether the effect of the acceptance of the disease on the emotional, cognitive and physical symptoms is moderated by the patients’ place of residence. Using a cross-sectional design, a total of 234 women with fibromyalgia were surveyed, of which 55.13% resided in rural areas and 44.87% in urban areas. Self-reported questionnaires were used to assess pain severity, (...) anxiety and depression, functional limitation, physical and mental fatigue and acceptance of the disease. The results show significant differences in acceptance, pain, and physical and mental fatigue between patients from rural and urban areas. The rural area patients presented higher levels of acceptance and pain and lesser levels of physical and mental fatigue compared to the urban area. The moderation analysis add that, only in patients from the rural area, the variables of physical symptoms were significantly and negatively associated with acceptance. This study addresses for the first time the role of the place of residence in suffering from fibromyalgia, suggesting that the rural or urban environment plays a relevant role in the severity and/or management of symptoms in fibromyalgia women. This work focuses on the application of machine learning techniques to assist the clinicians in the administration of analgesic drug during general anaesthesia. Specifically, the main objective is to propose the basis of an intelligent system capable of making decisions to guide the opioid dose changes based on a new nociception monitor, the analgesia nociception index. Clinical data were obtained from 15 patients undergoing cholecystectomy surgery. By means of an off-line study, machine learning techniques were applied to analyse the possible (...) relationship between the analgesic dose changes performed by the physician due to the hemodynamic activity of the patients and the evolution of the ANI. After training different classifiers and testing the results under cross validation, a preliminary relationship between the evolution of ANI and the dosage of remifentanil was found. These results evidence the potential of the ANI as a promising index to guide the infusion of analgesia. (shrink)

Understanding how culture evolves in society is an extremely difficult task. The bifocal stance theory (BST) deploys two copying strategies which can be linked to dual-system theories of behavior. BST would benefit from incorporating results from these theories, such as the evolution of attention to goals or steps of a behavioral sequence, and the role of the environment in prompting different copying strategies.

Tsirelson’s problem deals with how to model separate measurements in quantum mechanics. In addition to its theoretical importance, the resolution of Tsirelson’s problem could have great consequences for device independent quantum key distribution and certified randomness. Unfortunately, understanding present literature on the subject requires a heavy mathematical background. In this paper, we introduce quansality, a new theoretical concept that allows to reinterpret Tsirelson’s problem from a foundational point of view. Using quansality as a guide, we recover all known results on (...) Tsirelson’s problem in a clear and intuitive way. (shrink)

Several studies have explored differences between North American and European doctor patient relationships. They have focused primarily on differences in philosophical traditions and historic and socioeconomic factors between these two regions that might lead to differences in behaviour, as well as divergent concepts in and justifications of medical practice. However, few empirical intercultural studies have been carried out to identify in practice these cultural differences. This lack of standard comparative empirical studies led us to compare differences between France and the (...) USA regarding end-of-life decision-making. We tested certain assertions put forward by bioethicists concerning the impact of culture on the acceptance of advance directives in such decisions. In particular, we compared North American and French intensive care professional's attitudes toward: (1) advance directives, and (2) the role of the family in decisions to withhold or withdraw life-support. Student performance and its evaluation remain a serious challenge for education systems. Frequently, the recording and processing of students’ scores in a specific curriculum have several flaws for various reasons. In this context, the absence of data from some of the student scores undermines the efficiency of any future analysis carried out in order to reach conclusions. When this is the case, missing data imputation algorithms are needed. These algorithms are capable of substituting, with a high level of accuracy, the (...) missing data for predicted values. This research presents the hybridization of an algorithm previously proposed by the authors called adaptive assignation algorithm, with a well-known technique called multivariate imputation by chained equations. Depuis une vingtaine d'annees, on assiste un peu partout a un regain d'interet pour les ecrits socratiques de Xenophon. Que Xenophon ne nous donne pas davantage que Platon un portrait historiquement fiable de Socrate peut etre considere comme un acquis de la critique du XXe siecle. Laissant transparaitre dans son temoignage des options profondement differentes de celles de Platon, Xenophon temoigne par la meme, cependant, des tensions, voire des oppositions qui traversaient le milieu socratique autour du souvenir et de la (...) comprehension de la personne de Socrate: cette seule consideration suffit a faire de Xenophon une source essentielle pour notre connaissance de l'immediat apres Socrate et, par dela, pour notre intelligence de l'histoire de la reception de Socrate dans les siecles ulterieurs. Le colloque dont sont issues les etudes rassemblees dans ce volume, organise a l'Universite de Provence (Aix-Marseille I) du 6 au 9 novembre 2003, s'inscrit dans cette perspective. Plus qu'un bilan de la nouvelle orientation des etudes xenophontiennes, qu'il est encore trop tot pour etablir, on y verra l'illustration de la diversite d'approches dont est susceptible le temoignage de Xenophon sur Socrate. Due a Louis-Andre Dorion, une bibliographie des etudes sur les ecrits socratiques de Xenophon publiees dans le dernier quart de siecle temoigne de la vitalite retrouvee de ce domaine de recherche. Se presenta las aplicaciones desarrollas para virtualizar el laboratorio de fluidos y transferencias de la Universidad Metropolitana. El objetivo es permitir a los estudiantes realizar las prácticas de laboratorio de manera online. Se desarrolló una aplicación de cada práctica utilizando MATLAB App Designer. Los estudiantes lograron hacer cada práctica virtual manipulando en tiempo real las variables correspondientes y obtener los resultados con el cual realizar los informes. Estas aplicaciones fueron empleadas para dictar la asignatura de manera virtual, obteniendo buena aceptación (...) por parte de los estudiantes, los cuales sintieron cada práctica interactiva y de utilidad para su proceso de aprendizaje. The construct of the self is important in the domain of memory research. Recent work has shown that person memory is influenced by similarity of social targets to the self. The current experiments investigate self-similarity as defined by traits and political ideology to better understand how memory for social targets is organised. Across three experiments, participants formed positive or negative impressions based on each target’s picture, a trait-implying behavior (Experiments 1 & 2), and/or political ideology (conservative/liberal label in Experiment 2; (...) political-ideological belief statements in Experiment 3) followed by a memory test. Results showed a self-similarity effect dependent on valence in Experiment 1, but not in Experiments 2 or 3 when participants processed ideological information associated with targets. These results suggest that self-similarity has an effect on memory for social targets, but that ideological information disrupts self-focused processing of others, suggesting that ideological information also has a powerful influence on what people remember about others (i.e. social targets). Optimization, in general, is regarded as the process of finding optimal values for the variables of a given problem in order to minimize or maximize one or more objective function(s). Brain storm optimization (BSO) algorithm solves a complex optimization problem by mimicking the human idea generating process, in which a group of people solves a problem together. The aim of this paper is to present hybrid BSO algorithm solutions in the past 5 years. This study could be divided into two (...) parts: strategies and applications. In the first part, different strategies for the hybrid BSO algorithms intended to improve the various ability of the original BSO algorithm are displayed. In the second part, the real-world applications in the past five years in optimization, prediction and feature selection processes are presented. (shrink)

OBJECTIVES: To analyse the attitudes of medical personnel towards terminally ill patients and their right to be fully informed. DESIGN: Self-administered questionnaire composed of 56 closed questions. SETTING: Three general hospitals and eleven health centres in Granada (Spain). The sample comprised 168 doctors and 207 nurses. RESULTS: A high percentage of medical personnel (24.1%) do not think that informing the terminally ill would help them face their illness with greater serenity. Eighty-four per cent think the patient's own home is the (...) best place to die: 8.9% of the subjects questioned state that the would not like to be informed of an incurable illness. CONCLUSION: In our opinion any information given should depend on the patient's personality, the stage of the illness and family circumstances. Our study confirms that a hospital is not the ideal environment for attending to the needs of the terminally ill and their families. (shrink)

Manuel Alejandro Serra Pérez L’expression « virtus essendi » utilisée par Thomas d’Aquin dans certains passages de ses oeuvres a été un des sujets abordés dans le thomisme contemporain chez les plus importants commentateurs d’Aquin. Étienne Gilson fut le premier des commentateurs qui écrivit des notes explicatives sur son sens, montrant que ce pouvoir dont l’expression parle fait référence à la source d’où provient toute perfection et actualité, l’esse ut actus essendi. De son côté, le dominicain canadien Lawrence Dewan, (...) particulièrement intéressé à montrer que cette « virtus » est partagée par les deux co-principes de l’étant, à savoir, la forme et l’être, considère au contraire que la doctrine thomiste de la « virtus essendi » développe plutôt le thème que la forme est le principe de l’être (principium essendi). Face à ces deux lectures contraires, dans cet article nous proposons d’en faire une analyse systématique et de proposer une possible piste d’investigation qui éclaire le sens de ce que Thomas d’Aquin comprend par l’expression « virtus essendi ». Biobanks correspond to different situations: research and technological development, medical diagnosis or therapeutic activities. Their status is not clearly defined. We aimed to investigate human biobanking in Europe, particularly in relation to organisational, economic and ethical issues in various national contexts. Data from a survey in six EU countries were collected as part of a European Research Project examining human and non-human biobanking. A total of 147 institutions concerned with biobanking of human samples and data were investigated by questionnaires and (...) interviews. Most institutions surveyed belong to the public or private non-profit-making sectors, which have a key role in biobanking. This activity is increasing in all countries because few samples are discarded and genetic research is proliferating. Collections vary in size, many being small and only a few very large. Their purpose is often research, or research and healthcare, mostly in the context of disease studies. A specific budget is very rarely allocated to biobanking and costs are not often evaluated. Samples are usually provided free of charge and gifts and exchanges are the common rule. Good practice guidelines are generally followed and quality controls are performed but quality procedures are not always clearly explained. Associated data are usually computerised. Biobankers generally favour centralisation of data rather than of samples. Legal and ethical harmonisation within Europe is considered likely to facilitate international collaboration. We propose a series of recommendations and suggestions arising from the EUROGENBANK project. (shrink)

Headache disorders can be considered as the predominant neurological condition. In the field of neurological diseases, migraine was estimated to cost a total of €27 billion per year for the loss through reduced work productivity in the European Community. Medical data and information in turn provide knowledge based on which physicians make scientific decisions for diagnosis and treatments. It is, therefore, very useful to create diagnostic tools to help physicians make better decisions. This paper is focused on a new hybrid (...) clustering system combining analytic hierarchy process and weighted fuzzy c-means clustering method for diagnosing children with primary headache disorder. The proposed three-stage hybrid diagnosing system is tested on data set collected from hospitalized children in the Clinical Centre of Vojvodina, Novi Sad, Serbia. (shrink)

Le but de cet article est d'interroger les transformations culturelles qui accompagnent la commercialisation des inventions, phénomène majeur de l'essor marchand et consumériste au XVIIIe siècle. Les stratégies commerciales des inventeurs sont fondées sur une médiatisation croissante, mêlant rhétoriques visuelles (démonstrations, spectacles, expositions) et recours à l'imprimé: annonces de presse, affiches, prospectus, modes d'emploi, livrets d'utilisation. L'information et le savoir techniques jouent un rôle clef dans la construction de marchés pour les inventions. Cette dimension a rarement été prise en compte (...) dans l'histoire de la consommation et dans l'histoire des techniques. Pourtant, la littérature d'usage, qui postule l'autonomie et la capacité participative de l'acheteur comme argument de vente, exprime la montée en puissance des savoirs d'action, opératoires, prescriptifs, où priment le verbe et le geste, l'intention et le projet. Si la technologie des Lumières se déploie dans les traités et les dictionnaires, elle se manifeste aussi dans cette littérature éphémère, pratique et commerciale, source d'appropriations. (shrink)
